Application Form and Acknowledgement Receipt Submit along with the required documents.

IMPORTANT INSTRUCTIONS:
- Uploaded Birth Certificate should contain the child’s name issued by the Municipality .
- Uploaded Adhaar card of candidate size less than 1 MB.
- Uploaded Photograph of candidate size less than 1 MB.
- Uploaded Recent photograph of child along with Mother & Father size less than 1 MB.
- Incomplete forms and manually edited forms will be rejected and will not be called for
